Đặt vấn đề:
Gẫy xương đòn là một gẫy xương thường gặp, chiếm khoảng 2,5% – 5% trong tổng số gẫy xương nói chung và khoảng 10% - 15% trong tổng số gãy xương chi trên[4], [15].
Kết hợp xương được áp dụng phổ biến hiện nay để điều trị gãy xương đòn có mảnh rời ở các bệnh viện là đóng đinh nội tủy kết hợp buộc vòng chỉ thép. Khi rút đinh và lấy chỉ thép bệnh nhân sẽ có 2 đường rạch da để lấy chỉ thép và lấy đinh nội tủy. Chúng tôi đã mổ kết hợp xương đòn bằng đóng đinh nội tủy kết hợp buộc vòng chỉ Vicril đến khi liền xương chỉ có 1 vết mổ rút đinh. Để đánh giá phương pháp này chúng tôi tiến hành đề tài: “Đánh giá kết quả điều trị gãy xương đòn có mảnh rời bằng đóng đinh Kirschner nội tủy kết hợp buộc vòng chỉ Vicril” nhằm mục tiêu:
1. Đánh giá kết quả điều trị gãy xương đòn có mảnh rời bằng đóng đinh Kirschner nội tủy kết hợp buộc vòng chỉ Vicril.
Kết quả: 53 bệnh nhân được phẫu thuật có 98% liền vết mổ thì đầu, Xquang xương thẳng trục. 45 bệnh nhân được khám lại thì có 44 bệnh nhân liên xương tốt (97,8%), 1 bệnh nhân (2,2%) có cal xương xấu gồ ngay dưới da.
Kết luận: Phương pháp phẫu thuật kết hợp xương đòn bằng đóng đinh kèm buộc vòng chỉ Vicril có thể áp dung cho các bệnh nhân gãy xương đòn có mảnh rời
Evaluation of treatment results of clavicle fracture available fragments by the combinational method for both intramedullar drilling with Kirschner pins and fixation with Vicryl suture

ABSTRACT
Background:
A clavicle fracture is a common fractured bone and accounts for 2.5% - 5% of fractures in general, and about 10% - 15% of total fractures of upper extremities [4], [15].
Currently, the bone combination commonly applied to treat clavicle fractures available detached fragments in the hospitals is drilled by pins on both the broken ends of the clavicle and tied by surgical stainless steel suture. Patients will be 2 skin incisions to remove pins and surgical stainless steel suture. We have combined surgery procedure of clavicle by drilling intramedullarily and tied with Vicryl suture until bone fixation and there is only an incision for removing pins. To evaluate this method, we carried out the research: "Evaluation of treatment results of clavicle fracture available fragments by the combinational method of both intramedullar drilling with Kirschner pins and fixation with Vicryl suture " in order to gain the following aim:
1. Evaluating treatment results of clavicle fracture available detached fragments by the combinational method of both intramedullar drilling with Kirschner pins and fixation with Vicryl suture
Results:
53 operated patients accounting for 98% had the good closed surgery incisions at initial stage and bones straight axis on X-ray. 44 patients in 45 patients re-examined had fixed good bones (97.8%) and 1 patient (2.2%) had a bad bone healing bulging just below the skin.
Conclusion:
Method of collarbone combined surgery by drilling with Kirschner pins and tieing around with Vicryl suture can be applied patients with the collarbone fractures available detached fragments.
